Case Name The scald accident by the asphalt.

Place Kanagawa Prefect. Ayase City.
Location The asphalt factory.
Overview The worker closed the asphalt tank valve of the supplier while exchanging the gland packing of the switching valve( three-way cock ) of the asphalt piping. However,he was struck by 160° C asphalt gushing and he was burnt from his right wrist to the elbow, as he removed the metal hold packing without extracting the remaining pressure.
Incident The worker was burnt by overheated asphalt during pipe replacement work at the asphalt factory in Ayase City at about 11:30 on 8/10 in 1999.
Sequence 8:00 The KY meeting before the work.
11:00 The pipe replacement work started.
11:30 The accident happened.
Cause 1 Carring out works except for the KY meeting.
2 The victim had a lack of understanding about the danger of the exchange work.
3 The strainer ( filter ) had been placed under piping, and the valve of the pressure omission, which had been installed, was not used.
4 Though he had been taught that they should wear long sleeves work clothes while repair working, he wore the half sleeve work clothes on that day.
Countermeasures 1 They must absolutely connect to the employee and carry out the safe meeting during unplanned work.
2 Making the operation standard, and reeducating the workers.
3 Installing warning signs, etc., and catching a person's attention.
4 Making the work clothes standard.
5 Building in a changing order or reverse switch of the supplying pump.

Background 1 The recognition : doing the dangerous work without a meeting

Sequel We can see many cases in which during unplanned work accidents have occured.
