On that day the accident happened, back-filling and surface compaction work for west-side and north-side of sewage treatment facility, which began on that morning, were almost finished. Then at about 2:30 p.m., deciding to take the documentary photography, the victim started to prepare to do so in the place where drag shovel, which was in stopped standby state for leveling out the plot of land work, was at the back of him.Then the driver of the dump truck, which had carried earth and sands, intended to unload those. But as the stopped drag shovel lay in the way, he reversed the drag shovel about one meter and caught the victim, who was preparing photo shoots, under the crawler track. |
| Cause |
* The driver operated the drag shovel without any signal or guide of conductor.
* The driver did not realize the fact that there was the victim behind the drag shovel.
* Any measures to place off-limits control against the contact hazard in the running range of the drag shovel had not been taken. |

* You should forbid entrance to the operation range of the car type construction equipment and express clearly the prohibition zone with signboards, barriers, ropes, etc.
* If it is necessary to make a worker enter the operation range or running range of the car type construction equipment and carry out the work, you should station certain conductor and make the driver of the equipment run following the signal from the conductor. |
