A carpenter was cutting palster boards piled on the hole on the 2nd floor of new wooden house. He stepped back without care then missed his step, and he fell down to concrete ground 3m below from the entrance wellhole. |

Around 14:00, 7/20,in the construction field of new wooden house(2-floors), a carpenter missed his step and fell down to concrete ground 3m below from the entrance wellhole, and got wounded of conpulsory rest for 2 months |
| Sequence |
In the morning of the accident day, Directions of the share and arrangement of the work,and its contents were done by the president of the subcontractor on the telephone.
The supervisor of the contractor and the president of subcontractor directed the carpenter to fix the handrail on the wellhole, but they did not confirm that and left there.
The carpenter started to cut the plaster boards piled on the hole on the 2nd floor.
Around 14:30 , he stepped back without care, then missed his step and fell down to concrete ground of the 1st floor. |
| Cause |
The carpenter fell because he turned his back to the wellhole and worked and stepped back without care.
In the wellhole, the handrail to prevent fall was not installed.
The supervisor of the contractor and the president of subcontractor directed the carpenter to fix the handrail on the wellhole, but they did not confirm that and left there. |

In the entrance wellhole , the work must be carried out, after the handrail for the fall prevention is certainly installed. |
