Overview |
When they intended to hang a mud tank (about 1t estimation weight) using the crawler crane in the anti-erosion work field, this disaster overturned because of the overload. |

Sequence |
* At around 2:30 p.m., a field-worker A tried to discharge the mud in the mud tank by the crawler crane and drove it without permission, and stopped the crane diagonally backward right for the load. * It was turned toward the left about 45 degrees for the travelling direction in respect of the revolving super structure. * The jib length was expanded to the 8m which was the largest length, while the jib was sticked up to about 28 degrees, and the hook dropped. * The lift was tried, after slinging the water tank at one side in upper surface of it by wire and shackle. * The caterpillar raised a little with lifting it. * Since the engine of the crane stopped, A started it again. * Since the crane began to tilt with the starting of the engine, A got off the driver's seat, and he was hit by the turned crane, and he died. |
Cause |
* He intended to hang the load of the weight which exceeded the rated load by the crawler crane. * The ground in the installation site of crawler crane was weak and unstable. * Because the subgrade reaction of the crane in lifting concentrated and affected the ground, the inclination by the subsidence tended to take place. * The wiring of the safety equipment (the moment limiter) for preventing tipping over of crawler had been cut and did not work. |
Countermeasures |
* In the work using the transportable crane, the work plann is made beforehand based on breadths, landforms, geology, the ability of a use, a crane and is made known to everybody. * Because there are many cases which the work using cranes is carried out by the worker of many companies, it is important that the responsibility of each worker is established by clarifying the arrangement and chain of command of the workers. |

Background |
* The victim was engaged in the work of ordering of the field, etc. and did not have necessary the qualifications for the operation of the transportable crane and the slinging work. * The work was based on the plan which had not been decided beforehand, and the crane was driven with the thoughtless judgment of the field worker. |
Sequel |
* Because the enterprise made the disqualified person drive the transportable crane, the case was sent to the prosecutors in Industrial Safety and Health Law contravention. * After the accident occurrence, organizer took the license and drove the crane. |
