Overview |
This disaster is gravel decay in the soil pipe construction. The gravel against the steel plate decayed, after two steel plates were stood against the both sides of a reclaimed surface instead of sheeting sheet pile, and a worker who had entered the inside died. |

When the victim descended in the groove, after he drilled the groove in order to bury the soil pipe, and after standing two steel plates against the both sides of the reclaimed surface instead of sheeting sheet pile, and when it was formed between the steel plate, a worker who had entered the inside died. |
Cause |
* It was a condition that it was easy to decay because there was back-filled soil nearby the excavation position. * In spite of the possibility of the gravel decay, he came into the excavation groove. * The procedure of safe excavation is not known to everybody. * The operation chief did not sufficiently fulfill his job. * The investigation of the excavation position was not sufficient. * Safety and health management system was insufficient. * The safety and health education was insufficient. * The manager did not consider sufficiently prevention of the labor accident during construction. |
Countermeasures |
* They should adopt the method to that they can work without coming into the groove during the assembly of the sheeting timber support. * Work plans should be decided based on the result of the subsurface exploration. * The diagram is made beforehand in assembly of sheeting timber support. * The safety standard of working should be established. * The operation chief must direct the procedure of safe working. |
