When a worker stuck the gum tape for the curing on the handhole in front of the 2 ton dump truck at the sidewalk with the 3m width for pavement restoration before the start of the work, the driver did not notice him and started driving the dump truck forward. |

| Cause |
* It is the direct cause that the operator of the dump car did not check the front and rear before starting the car.
* The causes were recognition shortage of the designation, shortage of previous arrangement in morning meeting and KYK, and shortage of common knowledge of the operational procedure of the acting foreman, that he left the car without the lock and started without checking around there, the lack of the dangerous forecast, imperfection of nomination of representative person, and independent work with the arbitrary decision.
