Overview |
During tension work of PC girder (prestressed concrete), one of the wedges which were fixing 12 PC strand wires slipped, the wire projected out from PC steel and it stuck in the left thigh of the worker at the rear of opposite side jack. |
Incident |
Under tension work of the PC girder, the PC steel strand wire projected and it stuck in the left thigh of a worker. |
Sequence |
The tension work ended on the 2 PC cables out of 4 which placed in PC girder, and the third PC cable were being tensed. When tension work to be close to ultimate tensile force, it was found that the stroke of the tension jack wasn't sufficient , the jack couldn't reach fixed tensile force and dlongation quantity because of a failure of set of the tension jack. Therefore, the cable was to be fixed once and re-tension. When it set a tension jack to re-tension and began to increase the pressure, one of the cuneal which was respectively fixing 12 PC steel strand wire (φ 15.2mm) by the other tension jack, and one of the PC steel strand wire rushed in the opposite side out, and then it stuck in left foot femoral region of the worker at the rear of opposite side jack. |
Cause |
* The fitting of the wedge which fixed the PC steel strand wire in the tension jack was not accurately carried out based on the manual. * The victim worked in the dangerous position (rear of the tension jack). |
Countermeasures |
Retraining in how to do tension works. 1.The retraining in the operational procedure. 2.Preparation and inspection before the tension. |
