Overview |
During the replacement work of the road, the manager who had supervised the excavation work of a retaining wall was hit by vessel ( box ) which carry excavated soil sand lifted by cable crane, and the manager slipped 38m meters down the slope. |
Incident |
When the cable crane was operated, the vessel ( box for the excavated soil sand movement ) of the cable crane hit the manager who stood on the road shoulder in route where suspension load went sideways, the manager was shoved away, slipped down the slope and died. |
Sequence |
In the replacement work of the road, a worker A who excavated the retaining wall by drove a drag shovel repeated following work; he loaded the vessel with earth and sand which dug by his shovel, and after the vessel full he lifted it with a cable crane and carried it out to a dump space. Not long after the start of afternoon work, the manager came to the excavation field, directed work and watched. Then, since the vessel was full of excavated soil, worker A gave a signal of roll-up and traverse motion of a cable crane by using a transceiver to driver B. Driver B who received the signal manipulated the cable crane, lifted the vessel and move sideways about 5m, the vessel hit the manager who stood at road shoulder and shoved him away, height fell into the excavation field and slipped 38m down the slope. |
Cause |
As a cause of this disaster, it seems to following situation. 1. It was done in the place where the monitoring of the work situation was taking place, and was located right under the main ropeway. 2. In excavation work of retaining wall some work was carried out work by workers of other subcontractor, and the cable crane was working overhead of their working space. 3. The signal person gave the signal to the cable crane operator to operate without confirming safe situations of work and route in which the suspension load goes sideways. 4. During the cable crane was working, there was no role which forbid workers to enter the area under the lifted load come and go. 5. There was an insufficient prior examination for making work planning, etc.. 6. There was poor communication about adjustments for accident prevention, because many works carried out simultaneously. |
Countermeasures |
For homogeneous disaster prevention, it is necessary to develop countermeasures completely about the following points. 1. During a cable crane working, the route in which a suspensions load goes sideways is forbidden. 2. To clarify the route by fences, ropes or piles and carry out the display, in order to forbid the entrance to the route in which suspension load of the cable crane goes sideways. 3. Station a excusive signal person, and make him to watch keep out area and To carry out safe confirmation, station a exclusive signal person and to confirm safe conditions, and the signal to the cable crane driver is communicated. 4. To make work planning and examines countermeasure for ensuring safety of keeping out the area during the cable crane working, interruptions of the work, arrangements of the signal person, etc.. 5. The main contractor must carriy out communication adjustment for preventing labor accident which arises by carrying out work by a woker of the relation contractor intermingling in the identical place. 6. Safety education for the manager must be carry out for over safety management general in fields such as preparation of safety management plan and work planning of every process for field responsible person. |
