Overview |
In a construction of the purifying plant, the reinforced steel floor of the hydrograph room was longer than the design length. Therefore, after assembling the reinforced steel, the worker cut the end of the reinforced steel with a hand style reinforced steel cutter, and the offcut was sprung, and it hit his left eye. |
Incident |
On that day, when the worker did the assembly work with the reinforced steel floor of the hydrograph room, he noticed that part of the bottom reinforced steel was about 3 - 5cm longer than that of the design. He consulted with the foreman about that, and the foreman directed him to cut off the reinforced steel after assembling reinforced steel. So he decided that he cut the extra reinforced steel edge with the hand style reinforced steel cutter. He cut off the reinforced steel with the hand style cutter at the length of the design, but then the offcut was sprung and it injured his left eye by it. |

Cause |

Countermeasures |
* Before cutting work, the cutting place was held by a thick cloth, a net of a small mesh, or things to prevent a offcut from scattering. * Face protector should be used. * The dimension of reinforced steel is checked during the temporary assembly of reinforced steel. * Hand style cutter is not used with the thing which is expected to be sprung during cutting. |
