| Overview |
A worker walking on a road shoulder was run over by a dump truck going backward.
The cause was that an induction man did an induction, turning his eyes away from the dump truck with his back to it, though it went backward. Moreover, the induction man had not noticed the action of the victim on the side of the road shoulder. |
| Incident |
In the freeway, the victim was moving to the road shoulder 300m in front of the cutting point of the day in order to help clarify the position of the cutting point (assistant), following the instruction of the engineer.
There, in order to load cut asphalt, the dump truck went backward, following the signal of the induction, and ran over the victim. |
| Sequence |
* In the interchange, an engineer, 4 induction men, and 3 workers of cutting group had the meeting, and the engineer and induction men started the regulation about entering into these areas , and they finished it in 30 minutes.
* The engineer instructed the cutting group and the pavement group on the entrance about the regulations by portable telephone.
* The cutting group entered, and the entrance of the pavement group was delayed.
* The foreman moved for the photographing of cutting division before commencement of the work without waiting for the pavement group.
* The engineer requested the group leader of the pavement group to help indicate the position of the cutting point by portable telephone, and the victim ( 56-year-old ) was ordered to move through the road shoulder to the place with the engineer.
* Meanwhile, the induction man signaled to the dump truck of the asphalt transportation to move, because the cutting machine was attached to the fixed position.
* However, since the driver did not notice it, he signaled again by approaching the dump truck.
* Confirming the signal, the driver started to reverse the dump truck.
* The induction man did an induction in the direction of the cutting machine with his back to the dump truck
* The induction man exchanged the greeting with the victim halfway.
* The induction man heard the scream of the victim, when he approached the cutting machine. |
| Cause |
* The induction man did an induction with his back to the reversing dump truck.
* The victim and his group could not participate in the meeting, since the arrival of the pavement group was delayed.
* The engineer left the workplace for the photographing.
* Therefore, the engineer could not indicate the arrangement of workers at the start of the work.
* The driver of the dump truck lacked backward confirmation.
* The standby place of the dump truck was the road shoulder. |
| Countermeasures |
The operational procedure of the induction is made, and thoroughly well communicated to construction parties such as the induction man and drivers.
1. When the dump truck reverses, the driver must not go backward without an induction of the induction man.
2. The induction man does as follows: a. he tells the driver that he does the induction. b. In the position where the driver can see, 20 - 30m away from dump truck, turning to dump truck, he does the induction by using the stopping flag (guide light). c. he watch as well around the circumference without moving during the induction. d. When a dump truck came, the induction man makes the dump truck stop and wait.
3. The induction man repeats above-mentioned a - d and leads a dump truck to its destination.
4. A driver of dump truck must go, after he does the safe confirmation by himself by the rearview mirror, together with the induction of the induction man.
5. Crossing just before and after heavy machinery or truck comes is banned.
6. No one except an induction man must be between the cutting machine and the dump truck.
7. When crossing, a worker tells the induction man, and according to the indication of the induction man, he crosses.
8. The safety education of the above-mentioned matter is done.
9. The road shoulder is secured so that the worker may can pass. |
