Overview |
In the construction field, the victim and his colleague were carrying crushed stone from a dump truck by the backhoe, and overlaying and smoothing a subbase. When the victim approached the backhoe to pick something up, the backhoe suddenly moved, therefore the victim's left foot was run over. |
Incident |
The victim believed that the operator would interrupt the operation, if he approached, because they had always worked in the same group. |
Sequence |
On that day, the construction workers divided into two groups; subbase group and pavement construction group. At around 4 p.m., the pavement construction group joined subbase group. And at around 5 p.m., the victim was run over by the caterpillar of a reversing backhoe, when he was going to get the car in order to use it to clean the construction field. |
Cause |
* He entered within the operating radius of the backhoe. * He entered without making the signal to the operator. * He was convinced that the backhoe did not move, rather it had stopped. * The noise from the vehicles running on current road was so big that the backhoe movement was not audible. |
Countermeasures |
* Preparation and common knowledge of specific work planning. * When approaching a backhoe, we have to make the signal to an operator and confirm that the engine has stopped. * When working in the heavy machinery's operating radius, we need to station the induction man and have him manage there. * When working close to a backhoe, we should put something like plastic cones (recall mark made of the conical plastic, and are installed under road construction in order to draw workers' attention), distinguish the working area, and prevent workers from stepping into the area. |

Sequel |
The victim did not think that the backhoe moved backward, because the backhoe worked ahead, and he crossed the back of the backhoe in haste. In fact, he was slightly conscious of insecure. |
