Overview |
When the backhoe (0.2) was going downhill (about 2m level difference, 20-degree gradient) to put crushed stone into a vertical drain, the backhoe tumbled since it crossed the slope dangerously. The victim (operator) was thrown from the driver's seat with its action, and he was trapped under the roof bar of the backhoe to get injured. |
Incident |
The backhoe (0.4) had put crushed stone into a drain, but the bucket was so large, compared with the size of the slot (600 X 600), that the crushed stone seldom entered into the slot. The operator decided to switch the backhoe, and, on the way from the place upstream where another backhoe (0.2) was parked to the workshop, the operator drove it on the slope to cut corners. |
Sequence |
* After the victim and other workers had installed the net around 4 piles in the vertical drain, the crushed stone was put in by the backhoe (0.4) as was preliminarily decided. * The operator decided, on his own authority, to switch to a smaller backhoe (0.2), since the slot was so small that almost all the crushed stone was spilled and the work efficiency was bad. * The victim went to get smaller backhoe (0.2), and, in order to go back to the workshop by the backhoe (0.2), he drove along the road for the construction and turned over since he was crossing the slope near the workshop carelessly. |
Cause |
* Though the compaction of the slope was insufficient, the victim turned around on the top of the slope, went down on his own judgment, and turned over. * The victim selfishly changed the procedure of the work without doing the preliminary arrangement for the work change. * The work was left to the companies which had been cooperating with this company, because they had done the same work in the past. |
Countermeasures |
* We must prohibit the work which is attempted without preliminary arrangement; we have to do preliminary arrangement with main contractor in the case of the work change. * Work planning and the safe indication to the person involved are required. |
Sequel |
Though they worried that the disaster was a fatal accident, it turned to be a 9-days-lost-worktime disaster since his chest was not crushed. The reason why it was a minor accident was that the tip of the boom got on the top of the pile to measure the workshop. |
Incidental Discussion |
The victim could live, because a pile was at the point where the tip of the boom struck on the ground, when the backhoe turned over. |
