Case Name A dump trailer crashed into bridge girder when moving with its dumper raised

Location The river improvement works field
Overview The distance from the excavation point to the unload point was about 2.5 km, and a national road (second grade, 8 m width) was 0.5 km short of the unload point. The truck travelled with its dumper raised after unloading its burden, and crashed into the bridge girder.
Incident A dumper trailer crashed into bridge girder when moving with its dumper raised.
Sequence * The road was in such a good condition, being 10 m width and its surface having been improved, that the dump trailer could go at the speed of about 40 km/h.
* Though the colleague gave him some signal just before the accident occurrence, he did not notice it and continued regardless.
* The dump trailer crashed into the bridge girder and was damaged. On another front, the driver fractured his chest, and, as a result, he lost 4-weeks worktime.
Cause * The victim started driving without confirming the letdown of the dump trailer's bed.
* He missed his colleague's signal.
* He was using his cellar phone.
* He did not accustom himself to the field because he had entered the field just 4 days before.
* The driver was 56-year-old, and he was a little deaf.
* There was no such facilities telling "the vessel rising", just as a lamp, buzzer or rotation light.
* No signboard or portal gate in the dangerous place was installed.
Countermeasures * Drivers should set off after confirming the letdown of the trailer's dumper by pointing at it and calling it, etc.
* Warning equipment informing drivers of the raised dumper such as a warning sound and lamps are required.
* We should set a gate and an operator who is suposed to inform drivers of the condition of the dumper
