Overview |

Incident |
At around 16:48 on the 10th August, 2003, the victim who was carrying out the installation of the light sealing at the top of the second floor roof put his foot on the glass, and the glass broke, and he fell to the second floor, and hurt his head. |
Sequence |
* The victim entered the field with another person, and he attended the lecture for the newcomers. * The construction was scheduled to finish in two days, the 7th and the 8th. * However, it was impossible to work due to an approaching typhoon on 8th August, and the rest of the work was carried out on the 10th again. On that day, the workers arrived at the field at 8 o'clock in the morning, the removal work and the spare work of the sealing was carried out according to the instructions of the field director. |
Cause |
* There was no awareness of the danger that the top light might break (he removed the safety belt from the main rope carelessly and he moved). * This work was on the running board, and it was impossible to move on the stage plank while wearing the safety belt. * The instruction of the detailed procedure of the work on the top light was insufficient. * The net to prevent falling was not installed. |
Response |
