Case Name Glass which was standing toppled, and a worker was crushed under it.

Overview This accident occurred at the work which involved sticking handling description cards on a manual release box on the wall for smoke extraction. A worker intended to stick the description cards, keeping the glass apart from the surface of the wall without moving the glass, since the glass of the tenant interior member was stood in the work place. Then he couldn't support the dead weight of floated glass, and it fell on him. So he could not escape, and he was crushed under the glass.
Incident * The worker couldn't support the glass which stood against the surface of a wall, and he was crushed under the glass.
Sequence * It was a part of a tenant renewal work , and the work was to stick description cards on the workplace on the surface of the wall.
* At 9:35 a.m., the worker raised the glass ( 6 mm + 6 mm laminated glass, 1,660 mm X 1,100 mm and 14 others, gross weight 460 kg ) which stood on the manual box for smoke extraction in order to stick handling description cards on the box in a shop. Then, he intended to stick the description card as he supported the glass by his body. But he couldn't support the dead weight of the glass, and it fell on him. Then the glass knocked him over, and was sandwiched between the floor and the glass.
Cause * The worker failed to recognize the weight of the glass.
* He had also done other work, and there was no necessity of doing the work at that time. But he was able to cope with the work, as he controlled the other work.
* The installation work of the glass was the work of a different company, and he hesitated to speak to people of the different company, since the company was different, and he judged that it was easier to do it himself rather than to ask.
* The glass was stood against a wall.
* The place for the glass was restricted.
* The box for smoke extraction was not considered when the glass was placed.
* The prevention of toppling of the glass had not been done.
* No attention was paid to the glass placed temporarily.
* The temporary placing method of glass was not examined.
Countermeasures * If a situation is different from how it was descrived in the meeting, the worker doesn't judge for himself, and he reports to a site manager or a chief worker, and the procedure is decided, and the adjustment with other companies is done.
* Glass is not stood on a wall.
* In work and emplacement, a place which don't make any trouble for other comapnies is decided as a good place to put something.
* Glass is placed temporarily using the special tool, and the indication of attention is shown.
* Day and method of emplacement and temporary placing method adjusted to site execution are indicated.
* It is taught that the risk of a sheet of glass differs from that of more than one sheet of glass.
Knowledge Comment * Certainly a thing stood is toppled.
* The decline of the physical fitness ( physical ability ) is quicker than of the feeling.
* In the case of a different situation, "one's own judgement" is the origin of error !
Account of Concerned Parties The feeling that he wanted to advance the work gave priority to the others.
Reason for Adding to DB Since the same mistake is not repeated.
